Eligible health professionals include Primary Care Physicians, Primary Care Nurse Practitioners, Certified Nurse Midwife, Primary Care Physician Assistant, General Practice Dentist, Registered Clinical Dental Hygienist, Clinical or Counseling Psychologist, Psychiatric Nurse Specialist, Certified Social Worker, Mental Health Counselor, Licensed Professional Counselor, or Marriage and Family Therapist

Physicians and Dentists may receive up to $35,000 for a two year commitment and depending on available grant funds, $65,000 for a three year commitment.
Other qualified health professionals may receive up to $19,000 for a two year commitment and depending on available grant funds, $35,000 for a three year commitment.
