Up to $45,000 in total toward qualified educational loans over a one-to five-year period of service in six-month increments as follows: 6 & 12 mos service/$3,500;18 & 24 mos./$4,000; 30 & 36 mos/$4,500; 42 & 48 mos/$5,000; 54 & 60 mos/$5,500.

How to Apply:
|
Applications are made jointly by a physician and an organization or institution located in the community in which the physician wishes to practice. The application form contains sections for both the physician and the supporting institution/organization to complete. The physician applicant must submit loan verification information forms for each education loan submitted for repayment consideration. Only verifiable medical education loans qualify for the loan repayment benefit. Medical education debt may include undergraduate education and medical school, but may not include debt incurred during residency.
