Eligible Specialties:

Primary care physicians, dentists, psychiatrists, physician assistants, nurse practitioners and nurse midwives.

Service Requirements (formerly Service Stipulation):

Sign a contract with the Illinois Dept of Public Health agreeing to a minimum commitment of 2 years of service.

Who Can Apply:

US citizens, willing to practice on a full-time bases at a public or n/profit entity, licensed or approved to practice in Illinois. Must practice for a minimum of 2 years in a designated HPSA. Have a sponsor agree to contribute 50% of the annual award.

First 2 years of service/up to $25,000 annually; 3rd or 4th year/up to $35,000 annually.
