MDs:Doctors of Allopathic Medicine physicians must have any of the following training: family medicine, internal medicine, pediatrics, obstetrics/gynecology, or general psychiatry. General practitioners who have not completed residency training programs are not eligible.

PAs: Primary Care Physician Assistants
DDs: General Practice Dentists (DDS or DMD)
DHs: Registered Clinical Dental Hygienists
CNPs: Primary Care Certified Nurse Practitioners
CNMs: Certified Nurse-Midwives
CPs: Clinical or Counseling Psychologists
CSWs: Clinical Social Workers
PNSs: Psychiatric Nurse Specialists
MHCs: Mental Health Counselors
LPCs: Licensed Professional Counselors
MFTs: Marriage & Family Therapists

Service Requirements (formerly Service Stipulation):
|
Enter into a contract to provide direct patient care in a community health center in an underserved area, full-time for at least two consecutive years.
|

Who Can Apply:

US Citizen with valid CT license to practice; free of unserved obligations for service; free of judgments arising from Fed debt and agree to provide full-time primary care services for at least two consecutive years.

Up to $30,000 for first two year period; Up to $15,000 per any additional one-year extension.
